"A healer clone. Perfect for keeping allies alive through impossible odds."

Sam returns to the waiting room to find Caroline awake, staring at nothing with red-rimmed eyes that reflect fluorescent hospital lighting. She looks up when he settles beside her, reaching automatically for his hand.

"Any news?" Sam asks, though his enhanced hearing has been monitoring medical staff conversations for hours.

"Stage 3 aggressive lymphoma," Caroline whispers, voice cracking on words she's still learning to process. "Dr. Martinez says... says the treatment options are limited. Six months, maybe a year if chemotherapy works."

"In canon, Liz Forbes dies from cancer. Caroline loses her mother, gains vampire strength through grief, and spends decades carrying guilt about not being able to save the person who mattered most."

"Caroline," Sam says carefully, "what if there was a way to save her? Completely cure the cancer, give her decades more life, make sure she never gets sick again?"

"The System that gives me my abilities," Sam begins, taking her hands in his. "It just unlocked something called Gift of Power. I can transfer one of my supernatural affinities to another person permanently."

Caroline's eyes widen as she processes the implications.

"Transfer how?"

"My fourth clone has regeneration abilities. Accelerated healing, cellular repair, possibly even slowed aging. If I give that to your mom, the cancer dies immediately and she becomes nearly immortal."

"What's the cost?"

"Everything. The affinity disappears from me and my clone forever. One year cooldown before I can use Gift of Power again. If I ever need regeneration to survive a supernatural battle, it's gone."

"I lose that power forever. My fourth clone loses its special ability. And I can't use Gift of Power again for an entire year."

Caroline stares at their intertwined hands, tears streaming down her face as she processes the magnitude of what Sam is offering.

"You'd do that? Give up part of yourself to save my mom?"

Sam lifts her chin to meet his eyes, letting her see the absolute sincerity in his expression.

"For you? For your mother? Without hesitation, without regret, without a single second of doubt."

Caroline dissolves into sobs that carry relief and gratitude and overwhelming love in equal measure.

"Sam, I—"

"We tell her together," Sam interrupts gently. "But Caroline, this has to be her choice. I can't force supernatural abilities on someone who doesn't understand what they're accepting."

Twenty minutes later, they stand beside Sheriff Liz Forbes' hospital bed while she processes information that challenges everything she thought she understood about her daughter's boyfriend.

"Supernatural abilities," Liz repeats slowly, studying Sam's face for signs of deception or delusion. "You can create copies of yourself, and now you want to give me magical healing."

"Yes, ma'am."

"By touching my hand and... what exactly?"

"Transferring the regeneration affinity through the System's interface. You'd feel the cancer dying immediately as your body repairs itself at a supernatural level."

Sheriff Forbes has spent her law enforcement career evaluating impossible claims and distinguishing truth from elaborate lies. She looks at Caroline's desperate hope, Sam's calm certainty, and the medical chart that gives her six months to live.

"What are the side effects?"

"You'll heal from almost any injury short of decapitation or heart removal. You'll age much more slowly than normal humans. Your immune system becomes supernatural grade." Sam pauses. "And you'll be able to sense other supernatural beings the way vampires and werewolves can sense each other."

"So I become... what?"

"Still human. But enhanced human with abilities that let you protect people more effectively than any normal sheriff could."

Liz looks at Caroline, who nods through her tears.

"Do it," Sheriff Forbes says with the decisive authority that's defined her career. "If this works, I get to see Caroline graduate, go to college, get married, have children. If it doesn't work, I die anyway but with hope instead of despair."

Sam takes Liz Forbes' hand, opens the System interface, and selects Gift of Power activation.

Caroline hasn't let go of Sam's hand since her mother's healing, fingers intertwined with the desperation of someone who's just watched the person they love make an impossible sacrifice.

"Sam," Caroline says quietly, "you gave up something irreplaceable to save her. How do I ever repay that?"

"By being exactly who you are. By loving me despite the supernatural chaos that follows me everywhere. By choosing to stand beside me when Klaus arrives and everything goes to hell."

"You don't repay it," Sam replies honestly. "Love isn't a transaction. It's a choice we make every day to prioritize someone else's happiness over our own convenience."

Liz Forbes studies Sam with enhanced perception that reveals the truth beneath his calm exterior—the phantom pain from losing the Regeneration affinity, the exhaustion from reaching Level 10, the constant stress of protecting everyone he cares about from supernatural threats.

As they leave the hospital together—Caroline between Sam and her miraculously cured mother—Sam feels the weight of another life saved through impossible sacrifice. But he also feels the absence of Regeneration like a missing tooth, a constant reminder of the price he paid for love.
