Why do you hesitate to remove hemorrhoids when you know it won't be fatal?

The Spirit Realm feels 100% real, and any torture can be applied. Even after the torment, one can be healed with a Healing Skill and then continue the torture.
Compared to pulling teeth or removing hemorrhoids, it's nothing.
