Lily Shaw's injury wasn't as serious as it seemed. It was an urgent situation at the time; Simon Sawyer had gotten some medicine from somewhere, and it was really potent. She inhaled quite a bit when she entered without any precaution.
In order to protect herself, she had to gamble by choosing to self-harm, and fortunately, the result was in her favor. Simon, that pervert, actually wanted her alive and didn't want to see her self-harm.
But since it was her own thigh she targeted, she could control the force to some extent. The fruit knife had quite a bit of blood on it, but the wound wasn't as deep as expected.
