Though captivated by Ryuji's smile, Aoi Kanzaki, seeing him hesitate to undress, urged, "Hurry up! You've lost a lot of blood; the wounds must be deep. We can't delay!"
Ryuji glanced at his blood-soaked outer garment—stained with the blood of Lower Moon Five—and laughed. Aoi had clearly mistaken the demon's blood for his own injuries.
"What are you laughing at? Quickly take off your clothes so I can treat your wounds!" Aoi said, a touch impatiently.
Before Ryuji could respond, Aoi started to undress him herself.

Zenitsu, witnessing Aoi undressing Ryuji, became excited and started teasing, "I saw that! You two are doing this in broad daylight!"

Ryuji caught Aoi's hands, stopping her. Ignoring Zenitsu's threat—the guy always acted this way when a girl showed him attention, immediately using Amane Ubuyashiki to try and intimidate him—Ryuji explained, "I'm a medical attendant at the Butterfly Mansion, and I need your cooperation."
Aoi, surprised by Ryuji's lack of cooperation, responded firmly, "I am a medical attendant at the Butterfly Mansion specializing in treating injuries. Please cooperate."
"I'm not injured. The blood on my clothes isn't mine; it's demon blood. You should examine Tanjiro first," Ryuji explained.
Aoi paused, then changed her approach, "Even so, I still need to examine you."
Zenitsu, watching Ryuji being "forced" by a young girl, was secretly thrilled and envious, fantasizing about a girl being this assertive with him.
Unable to dissuade Aoi, Ryuji allowed the examination, but insisted on undressing himself.
