The restraint from interfering is simply to better control the setup and pacing, not to bind themselves with limitations.

Delicate Extraction is even more complex. Within all of Konoha, perhaps only Tsunade, Nonō Yakushi (Kabuto's adoptive mother), and at most three others have reached that level.
And this is in Konoha—the strongest village when it comes to medical ninjutsu.
That means fewer than five elite medical-nin exist with such surgical capability.
