In larger organizations, especially public hospitals or government sectors, there's a principle of avoiding nepotism. For example, if the husband is the hospital president, the wife being the vice president is generally not allowed.
The law clearly stipulates that spouses, direct relatives, collateral relatives within three generations, and close in-laws cannot hold positions subordinated to the same leader or be in direct superior-subordinate relationships. Moreover, they can't engage in organizational, personnel, disciplinary, auditing, and financial work in the same institution where either party holds a leadership position.
So in major hospitals, a president and vice president being family members is almost non-existent.
Unless it's a privately run hospital, in which case no one can interfere.
